PO-02-059 INFLAMMATORY REACTION TO ANTIBIOTIC ELUTING POUCH MASQUERADING AS RECURRENT CARDIAC IMPLANTABLE ELECTRONIC DEVICE INFECTION

Absorbable antibacterial envelopes mitigate risk of pocket infections during repeat cardiac implantable electronic device (CIED) procedures. Despite use envelopes, CIED infection persists. However, hallmark signs including incisional erythema, drainage, and wound dehiscence overlap with a less common clinical entity – allergic reaction. To present case recurrent secondary to an inflammatory response the envelope proven histological evaluation. N/A A 52-year-old male remote aortic valve replacement complicated by complete heart block dual chamber pacemaker (DC PPM) underwent generator change Tyrx pouch. One week later, he developed partial cloudy drainage. The patient was started on antibiotic therapy percutaneous system extraction placement externalized right ventricular lead. Pocket cultures were negative. contralateral DC PPM inserted completed course antibiotics. Within two weeks placement, presented dehiscence. new removed pathologic specimen obtained from presumably infected pacing lead placed. Cultures again demonstrated foreign body lymphoplasmacytic focal neutrophilic inflammation most consistent implantation low-lateral left-sided distant prior site without envelope. discharged antibiotics no evidence or at over follow up period 6-months. Allergic reaction should be considered dehiscence, particularly if recurrent. Tissue biopsy can elucidate mechanism.
